Whether you are volunteering in ministry for the first time or continuing your involvement, please complete and return this form.  Returning your form in a timely manner provides current information for recordkeeping and ministry planning. 

Ministry commitments are in effect for approximately one year.        

(A reminder for St. Mary School families: completion of this ministry form and serving in parish ministry are qualifications for receiving parish subsidy towards tuition.)

Important Notice

 

To ensure the safety of parish youth and vulnerable adults who have been entrusted to our care, St. Thomas More Church complies with Diocese of Owensboro Safe Environment requirements.  Adults age 18 and older who serve in a parish ministry or program in a position of authority, trust, or supervision involving contact with minor children or vulnerable adults must have current Safe Environment Certification and a current background check from the Diocese of Owensboro.
